​​​​​What is covered by my insurance?

Insurance benefits are verified prior to each visit at Northwest Pain Specialists. We accept most commercial insurance plans as well as some state insurances, and can provide information regarding how these plans will process. Please bring your current insurance information to your appointment. 

 

Referral information

We do require a referral prior to scheduling a new patient consult. Please contact your primary care provider or another health care professional to have them send us your referral. Depending on your insurance we may need prior authorization before doing any procedures. 

Insurance benefits and authorizations

​In order for insurance companies to authorize a procedure, it must be deemed medically necessary and reasonable. If it is not determined as such, the service will not be covered by your insurance. Our office will work hard to ensure that every effort is made to obtain these authorizations.
